I need your contact details, which include name, address, postcode and phone number. I also need information about your health, lifestyle and age for treatment to go ahead.
I need all the information to provide a safe and suitable treatment, and to allow me to compose a tailor-made treatment plan.
I must also comply with the professional code of conduct and ethics set out by the Association of Reflexologists (AoR). I also need this information for insurance purposes.
I must retain records for a period of at least 7 years from the date of your last treatment. In a minors case this would be 7 years after their 18th birthday.
Jacqueline Adams will not share your information with any third parties. I am committed to ensuring that your personal data remains secure.
I will contact you using the contact preference that you give me during our first consultation. This will be in relation to appointment times.

If you don’t agree to your therapist keeping records of information about you and your treatments, or if you don’t allow them to use the information in the way they need to for treatments, the therapist may not be able to treat you.
Your therapist must keep your records of treatment for a certain period as described above, which may mean that even if you ask them to erase any details about you, they might have to keep these details until after that period has passed.
A copy of this document will be sent to you should you contact me via email, and a hard copy will be provided at our first consultation in order to confirm that you have understood and accepted my privacy statement and your rights under the GDPR.
